Membership and costs

What is concierge medicine?

Concierge medicine is membership-based primary care. The practice limits how many patients it serves and charges a flat membership fee. In return, visits are longer, access is faster, and your care team actually knows you.

How much does membership cost?

Core Concierge is $450 a month and Premium Longevity is $700 a month, each with a one-time $250 enrollment fee. The first fifty founding members join at $350 and $595, locked for as long as they stay, with enrollment waived. Each additional family member joins at 15 percent off, limited to four per household. We walk through everything each plan includes during your consultation, before you commit to anything.

Do you accept insurance?

Membership covers your care relationship with our practice. Most members keep their health insurance for specialists, imaging, hospital care, and prescriptions. We are happy to explain exactly how it works with your coverage during your consultation.

What if I am healthy and barely use it?

Most of our members are healthy, and that is the point. Membership is not a punch card. Every member uses the annual comprehensive assessment and the preventive plan built from it, and the access is worth most on the day you unexpectedly need it.

Your care team

Who oversees clinical care?

360 Health Center is a physician-owned medical practice. Clinical protocols, oversight, and referral pathways are structured the way a medical office is structured, not the way a wellness storefront is. If a question needs a specialist, we say so and help arrange it.

How do I know this is not wellness hype?

Judge us on what we publish. Our pricing is on this website rather than behind an inquiry form. We name what each membership includes before you commit. And we are direct about what we will not sell you: no gray-market compounds, no guaranteed outcomes, and no treatment we cannot explain the evidence for.

Can you coordinate with my specialists?

Yes. Care coordination is part of the membership. We help arrange specialist visits, share the relevant history, and stay involved so nothing falls through the cracks.

Who will I see at my appointments?

You will see the same small team at every visit, led by Madison Spina, our nurse practitioner, so the person in the room already knows your history.

Appointments and visits

How quickly can I be seen?

Same-day and next-day appointments are standard for members. That is one of the main reasons our membership is limited.

What happens at my first visit?

Your first visit is a thorough review of your health: history, medications, risk factors, and goals. You leave with a plan and a direct way to reach us.

Is 360 Health Center taking new patients?

Membership is limited by design, and availability changes as the practice grows. Request an appointment and we will let you know current availability right away.

Where are you located?

We are located in Cresskill, New Jersey, and welcome patients from across Bergen County and the surrounding area.
